2. Sales Tax, One-time payment, and Down-payment: If you reside in the State of New York in the United States, you are required by law to pay the New York State sales tax, which is 8.875% or $146.44. If you are in New York State and want to buy your ionizer on the installment plan, your down-payment has to include the full sales tax.

3. Method of Payment: In order to keep the cost of the ionizer reasonable, the only method of payment available at this time is by check or money order made payable to “Chenax Majesty IP, Inc.”

If you want to buy your Robusta 8 ionizer on the installment plan, you will, with your down-payment, include 3 or 4 post-dated checks for the remaining balance. Please send your checks to Chenax Majesty IP, Inc., 443 Greene Avenue, Brooklyn, NY 11216. Your ionizer will be shipped after your down-payment or single-payment check has cleared.

Chenax Majesty will notify you by e-mail 7 days before each installment is due for deposit. There is a $35 processing charge if your check is returned unpaid by your bank.

The Lay-away Plan: With a lay-away plan, you can make owning a Robusta 8 water ionizer even easier. Complete the forms and open your easy lay-away plan by sending $100 or more. Then send what you can, when you can. Once your deposits add up to the down payment, we will notify you and ask you to send in the monthly installment checks. As soon as we receive your installment checks, we will ship your Robusta 8 to you.

4. How to Order and Register:
4a. View, download, and complete the 2 or 3 forms you need. (See links in the blue table, below.)
4b. Make your checks or money orders payable to Chenax Majesty IP, Inc.
4c. Send the forms you signed and the check/s to Chenax Majesty IP, Inc., 443 Greene Avenue, Brooklyn, NY 11216.

• 4-1. Complete and sign the form titled “HealthiLifer Ionizer Order Form (Individual) and Optional Application For HealthiWater Representative”. If you do not know your HealthiWater Sponsor’s Id .No., please telephone him/her to obtain the number. We cannot accept an order without a sponsor.
• 4-2. Complete and sign the form titled “HealthiLiferIP Ionizer Guarantee, Activation, and Claim Procedure Effective January 1st, 2011”. This form locks in for you the strongest money-back guarantee in the ionizer industry.
• 4-3. You will need to complete and sign the US government Form W-9, titled “Request for Taxpayer Identification Number and Certification”, if you want to also become a HealthiWater Representative and Sponsor (or a Member of the HealthiLifer Wellness Club who wants to sponsor other people into becoming members). You do not need to complete Form W-9 if you just want to buy ionizers. In Form W-9, you provide personal or corporate information for income tax purposes. You will get and complete the pdf Form W-9 online, and then print and sign the first page. To do so, click on the link in row 3 of the table below.
• 4-4. Mail the original forms to Chenax Majesty IP, Inc. at 443 Greene Avenue, Brooklyn, NY 11216. Then fax or mail a photocopy of only the completed “HealthiLifer Ionizer Guarantee, Activation, and Claim Procedure” form to your HealthiWater Sponsor. Do not send any other form to your Sponsor. You should save a copy of all forms for your records.
• 4-5. After your order and application are processed, Chenax Majesty IP, Inc. will e-mail you your own HealthiWater Sponsor Id. Number.
